Community of Hope is a mission-driven nonprofit organization that has been providing healthcare, housing, and supportive services to low-income, underserved, and homeless individuals in Washington, D.C. for over 40 years.

About the Role

We are seeking an experienced Clinical Nurse Manager to lead our clinical team at our Conway Health & Resource Center in SW DC. This full-time position offers a competitive salary of $95k and up based on experience.

Main Responsibilities
  • Oversee the development of patient-centered individual care plans to ensure coordinated care across healthcare providers, settings, conditions, community service providers, and caregivers.
  • Supervise nursing and medical assistants at designated sites, ensuring high-quality care and smooth patient flow.
  • Assist with chronic care management by providing education and support to patients.
  • Implement controlled substance regulations and comply with OSHA and safety guidelines.
Requirements
  • Bachelor of Science degree in Nursing required.
  • Current, unencumbered DC Registered Nurse license with current CPR certification required.
  • One year of supervisory experience and one year working in an ambulatory, primary care or med/surg setting.
