LVN Care Coordinator

About the Role
Titanium Healthcare, Inc. is seeking a skilled LVN Care Coordinator to join our Enhanced Care Management team. As an LVN Care Coordinator, you will play a vital role in coordinating care for our members, ensuring they receive the best possible support and services.
Key Responsibilities
- Facilitate access to primary care and behavioral health providers as needed
- Work effectively with the interdisciplinary team to provide high-quality care management to ECM members
- Engage members in skilled therapeutic interactions to promote health behaviors and other behavioral health interventions
- Coordinate care, including resource linkages, with other professionals and organizations in the community
- Ensure seamless transitions of care that meet IEHP's requirements for post-ER visits and hospitalizations
- Complete regularly scheduled follow-up visits
- Provide care coordination through phone calls, emails, messages, updating calendars, scheduling appointments, data entry, and attending meetings
- Support BH Care Manager, Community Health Worker, and Care Coordinator with delegated tasks
- Collaborate on member care issues with other ECM care team members
- Participate in weekly systematic case reviews and ad hoc case reviews
- Consult with the Clinical Nurse Leader and/or the Behavioral Health Care Manager before taking any clinical action
- Strategize to engage members in care and conduct motivational interviewing
- Provide health education to members within the scope of practice
- Initiate a care plan and complete all necessary assessments per healthcare plan guidelines
Requirements
- High School Diploma/GED
- Minimum 2 years of experience as an LVN working in a healthcare setting, such as a medical clinic or acute care hospital
- Experience with an EHR/EPM
- A valid California driver's license and clean driving record
- A reliable vehicle and valid automobile insurance
- A valid, unrestricted state LVN license
