About the Role:The Home Nurse Visitor will work closely with a multidisciplinary team to provide comprehensive care to women and their families, focusing on maternal and child health, nutrition, and family planning. The successful candidate will have a strong background in maternal/child health, pediatric health, or a closely related field, with a minimum of two years of full-time work experience in a public health, community-based, hospital, clinic, or home visiting setting.
Responsibilities:- Assess the physical, emotional, social, and environmental needs of women and their families, and develop a plan of care to address these needs.
- Provide counseling and education to women and their families on topics such as prenatal and postpartum care, nutrition, parenting, childcare, family planning, and special health problems.
- Link women and their families with community resources and services that meet their specific needs.
- Collaborate with other healthcare professionals to ensure comprehensive care and support for women and their families.
- Document nursing activities and maintain accurate records in accordance with project and institutional guidelines.
- A license and current registration to practice as a Registered Professional Nurse in New York State.
- A Bachelor of Science degree in Nursing from a regionally-accredited college or university, or one recognized by the New York State Education Department as following acceptable educational practices.
- A minimum of two years of full-time work experience in maternal/child health, pediatric health, or a closely related field, in a public health, community-based, hospital, clinic, or home visiting setting.
