RN Care Team Coordinator

An RN Care Team Coordinator works within the interdisciplinary team to create comprehensive individualized plans of care for assigned patients. The ideal candidate will have a strong background in nursing and excellent communication skills.
Key Responsibilities:- Develop and implement individualized plans of care in collaboration with the interdisciplinary team.
- Collaborate with the interdisciplinary team to ensure coordination of services.
- Participate in Continuous Quality Improvement activities to support the quality of Ohio's Hospice services.
- Precept new staff to support professional relationships with newly hired team members.
- Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred.
- Active Registered Nurse license in the State of Ohio without any board actions.
- Two years acute care nursing experience preferred.
- Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment.
