Job Overview

The Medicaid Enrollment Coordinator plays a crucial role in supporting individuals as they navigate the application process for Long Term Supports and Services (LTSS) Medicaid.

Key Responsibilities:
  • Facilitate case management by aiding clients in addressing their personal, social, health, and economic requirements.
  • Implement a person-centered planning approach to engage and assist individuals in accessing services that enable them to remain in their homes.
  • Guide clients through the process of applying for LTSS Medicaid and provide necessary referrals.
  • Manage a caseload while assessing initial and ongoing eligibility for various services.
  • Ensure timely completion of tasks and respond to inquiries within a specified timeframe.
  • Utilize company databases and tools for effective case management and tracking.
  • Proactively check for new referrals and maintain communication with participants to facilitate application completion.
  • Oversee the application process from initiation to decision, including the collection of required documentation.
  • Perform additional duties as assigned by management.
Required Knowledge, Skills, and Abilities:
  • Understanding of social work principles, practices, and methodologies.
  • Familiarity with Federal and State eligibility and assistance criteria.
  • Awareness of community resources and agency eligibility standards.
  • Proficient in drafting objective narratives and maintaining accurate logs and reports.
  • Skilled in interviewing clients and assessing their needs effectively.
  • Able to build and maintain professional relationships with colleagues and clients.
Qualifications:

Candidates should possess education, training, or experience that demonstrates proficiency in the following areas:

  • An Associate's degree or higher in Behavioral Science or a related discipline.
  • A minimum of six months of experience in developing recommendations for client service plans, including clinical treatment or eligibility assessments.
  • Experience with automated information systems for data management and reporting.
  • At least six months of experience in case management.
  • Strong narrative writing skills.
