Job Summary

We are seeking an experienced Care Coordinator to join our team at Liberty Health. As a key member of our interdisciplinary team, you will be responsible for providing comprehensive psycho-social assessments and developing patient-centered care plans that address the unique needs of our patients and their families.

Responsibilities
  • Conduct thorough assessments to identify social, emotional, financial, and environmental factors that impact the medical plan of treatment and desired outcomes.
  • Develop and maintain individualized care plans that incorporate evidence-based interventions and address the physical, emotional, and spiritual needs of patients and their families.
  • Engage in critical thinking and problem-solving skills to address complex issues and develop creative solutions that promote positive outcomes.
  • Collaborate with healthcare professionals, community resources, and family members to ensure seamless transitions and continuity of care.
  • Maintain accurate records and documentation of patient interactions, assessments, and care plans.
  • Participate in regular meetings and updates with the interdisciplinary team to share knowledge, best practices, and concerns related to patient care.
Requirements
  • Bachelor's degree in Social Work or related field; Master's degree preferred.
  • At least one year of experience in healthcare, preferably in a hospice or palliative care setting.
  • Strong communication, interpersonal, and problem-solving skills.
  • Ability to work independently and collaboratively as part of a team.
  • Flexibility to adapt to changing situations and priorities.
  • Basic computer skills and proficiency in electronic health record systems.
Work Environment

This position requires occasional travel within Buncombe and Haywood counties. Regular evening and weekend hours may be required. Candidates must possess a valid driver's license and reliable transportation.

Compensation

The estimated salary range for this position is $ 65,000 - $85,000 per annum, commensurate with experience. We offer a competitive benefits package, including medical, dental, and vision insurance, 401(k) retirement plan, paid time off, and continuing education opportunities.
