Job SummaryThe Care Coordinator is a Registered Nurse who plays a critical role in facilitating the plan for inpatient/outpatient care. This position requires a collaborative and interdisciplinary approach to ensure seamless coordination of clinical care and discharge planning services.
Key Responsibilities- Collaborate with the interdisciplinary team to assess patient needs and develop a comprehensive plan of care.
- Act as an advocate for patients, physicians, and hospital staff to ensure compliance with federal, state, and third-party utilization management regulatory requirements.
- Focus on anticipation, integration, and coordination of clinical care and discharge planning services to optimize patient outcomes.
- 3 years of experience in a nursing-related area, with a focus on clinical case management, utilization management, or home/community health.
- Graduate nursing degree required; Bachelor's degree in nursing preferred.
- Registered Nurse licensure required.
