Care Coordinator RN

Job Summary:
The Care Coordinator RN is a registered nurse who facilitates the plan for inpatient/outpatient care by using a collaborative interdisciplinary approach. This role focuses on anticipation, integration, and coordination of clinical care and discharge planning services to meet the needs of patients and optimize clinical and resource outcomes.
Key Responsibilities:
• Collaborate with the interdisciplinary team to assess patient needs and develop the plan of care.
• Act as an advocate for the patient, physician, and hospital relative to compliance with federal, state, and third-party utilization management regulatory requirements.
• Focus on anticipation, integration, and coordination of clinical care and discharge planning services.
Requirements:
• 3 years of experience in a nursing-related area, with clinical case management, utilization management, or home/community health experience preferred.
• Graduate nursing degree required, with a bachelor's degree in nursing preferred.
• Registered nurse licensure required.
Work Schedule:
Monday, Tuesday, and Wednesday, 2pm to 10pm. No weekends or call requirements.
