Job Summary

Summit Health, Inc. is seeking a skilled Registered Nurse or LPN to join our multidisciplinary care team as a key member. In this role, you will work closely with our primary care providers, clinical practice managers, and office supervisors to deliver exceptional patient care and support.

Key Responsibilities
  • Assist primary care providers with daily patient care coordination and physician access.
  • Function as a liaison between patients, staff, and physicians to ensure seamless communication and care.
  • Maintain efficient patient flow and prioritize patient care by implementing physician orders and nursing procedures.
  • Deliver direct patient care within the scope of licensure and in accordance with Summit Health, Inc. policy.
  • Collect lab specimens, perform Point of Care testing, EKGs, and phlebotomy as applicable.
  • Prioritize patient care, manage nurse visit schedules, and prepare pertinent information for patient visits.
  • Effectively communicate problems, concerns, or issues to the Office Supervisor/Manager promptly.
  • Assure all patients are correctly identified prior to providing and/or documenting care.
  • Employ appropriate and timely use of Tasking in EHR.
  • Manage Clinical Nurse/Assigned inbox.
  • Obtain vital signs, administer injections, and perform patient outreach to satisfy quality measures.
  • Coordinate and facilitate daily huddles.
  • Appropriately demonstrate telephone triage protocols and document in EHR.
  • Schedule appointments and identify and schedule annual visits as applicable.
  • Monitor daily hospital discharge list and collaborate with Care Management for TCM appropriate visits.
  • Adhere to universal safety precautions when administering medications and disposal of medical waste.
  • Review and update medication lists in EHR and demonstrate understanding of prescription control and prescription refill procedures.
  • Initiate and follow through with prior authorization requests, DME orders, and home health referrals.
  • Enter orders and schedules appointments in the Practice Management System (PMS) and EHR.
  • Provide referrals and/or consult orders appropriately and in a timely manner in the PMS and EHR.
  • Check and maintain logs for emergency equipment according to policy.
  • Facilitate transition of care to UCC/Hospital.
  • Maintain proficiency with department-specific equipment, troubleshoot, and report malfunctions promptly.
  • Actively participate in pilot programs.

Education, Certification, Computer, and Training Requirements
  • Associates degree required. Bachelor's degree preferred.
  • New Jersey State Nursing License and BLS certification required.
  • 0-1 years experience. 2-4 years experience preferred.
  • Ability to communicate in English, both orally and in writing.
  • Strong interpersonal and organizational skills.
  • Ability to perform diverse work assignments with time limitations with a high degree of accuracy.
  • Ability to use problem-solving, critical thinking, and priority-setting skills.
  • Experience with Standard Office Equipment (Phone, Fax, Copy Machine, Scanner, Email/Voice Mail) preferred.
  • Experience with Standard Office Technology in a Window-based environment preferred.
  • Experience with patient care equipment preferred.
  • Experience with EHR, Practice Management System, and Departmental/Ancillary Systems preferred.
