The Senior Community Care (SCC) program is a continuous, integrated care delivery initiative that coordinates clinical care for members in facility settings. The Advanced Practice Clinician (APC) within SCC focuses on our highest-risk health plan members and collaborates with an interdisciplinary team.

Key Responsibilities:

  • Perform thorough assessments
  • Oversee members during acute medical situations or exacerbations of chronic conditions, including necessary testing and interventions
  • Support members through short-term sub-acute rehabilitation
  • Ensure precise and comprehensive documentation, including ICD-10 codes
  • Collaborate and communicate effectively with the interdisciplinary care team
  • Engage in advanced illness and care planning discussions
  • Offer counseling and education to patients and their caregivers

Required Qualifications:

  • Education:
  • NP: Graduate of an accredited Master of Science in Nursing or Doctor of Nursing Practice program
  • PA: Graduate of an accredited Physician Assistant program
  • Active and unrestricted license in your state or ability to obtain by start date
  • Active Nurse Practitioner or Physician Assistant certification through a national board:
  • NP: Board certified by the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following:
  • Family Nurse Practitioner
  • Adult Nurse Practitioner
  • Gerontology Nurse Practitioner
  • Adult-Gerontology Acute Care Nurse Practitioner
  • PA: Board certified by the National Commission on Certification of Physician Assistants
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, as per state regulations
  • Ability to meet the physical demands of the role, including lifting a 30-pound bag, navigating stairs, and performing physical assessments
  • Ability to establish a collaborative practice agreement, if applicable in your state
  • Access to reliable transportation

Preferred Qualifications:

  • 1+ years of clinical experience in practice (long-term care setting preferred)
  • Experience addressing the medical needs of patients with complex behavioral, social, and functional requirements
  • Knowledge of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end-of-life discussions
  • Proficient in computer skills, including documentation in written and electronic medical records

New York Residents Only: The salary range for this position is $88,000 to $173,200 annually. Compensation is determined by various factors, including local labor markets, education, work experience, and certifications. UnitedHealth Group adheres to all applicable minimum wage laws. In addition to salary, we offer a comprehensive benefits package, incentive and recognition programs, equity stock purchase options, and 401k contributions (subject to eligibility requirements).
