Position Overview

Certified Nurse Midwife (CNM)

Location – Duke University Health System

Role Summary

The Certified Nurse Midwife (CNM) serves as an advanced practice provider (APP) delivering comprehensive patient care services across various stages of maternal health, including antepartum, intrapartum, postpartum, and gynecological care. This role encompasses both primary and specialized care across diverse settings, including inpatient units and outpatient clinics. The APP is responsible for conducting physical assessments, gathering and documenting clinical data, performing diagnostic and therapeutic procedures, and prescribing necessary interventions and medications while ensuring coordinated patient care and discharge planning.

Work Schedule

APPs typically engage in an average of 40 hours per week, which may include both inpatient and outpatient responsibilities. Outpatient duties are scheduled Monday through Friday, with options for 4 or 8-hour shifts. Inpatient responsibilities may vary from Sunday to Saturday, with 8 or 12-hour shifts, subject to modifications by leadership to align with clinical needs.

Key Responsibilities

Clinical Expertise

  • Collaborate with attending physicians and the healthcare team to assess, diagnose, plan, prioritize, and evaluate patient care effectiveness and efficiency.
  • Provide and coordinate clinical care for patients under the OB service, adhering to established protocols.
  • Conduct daily rounds to evaluate patient status, initiate treatment plans, and assess the effectiveness of obstetrical care.
  • Engage in evidence-based clinical practice by reviewing relevant literature.
  • Monitor patients for changes in condition and implement appropriate interventions, including emergency measures as necessary.
  • Communicate all pertinent patient information effectively to the healthcare team.
  • Document all relevant clinical data, including diagnostic test results and patient responses to interventions.
  • Lead and coordinate care for the OB-GYN service.
  • Utilize prescriptive authority in accordance with organizational and regulatory guidelines.
  • Exhibit advanced nursing skills and evidence-based practice.

Education and Training

  • Identify educational needs of nursing staff and healthcare team members, providing both individualized and formal training as required.
  • Develop and assess educational programs for nursing staff and other healthcare providers.
  • Mentor healthcare learners through structured agreements.
  • Evaluate the educational needs of patients and families, delivering culturally sensitive interventions.

Research and Quality Improvement

  • Apply current research and evidence-based practices in clinical settings.
  • Incorporate research findings into the development of care standards.
  • Support approved clinical research initiatives through various activities, including patient consent and outcome reporting.
  • Participate in interdisciplinary quality improvement and research efforts.

Consultative Role

  • Provide expert guidance and resources to healthcare providers, patients, and the community regarding maternal and fetal health.
  • Develop and implement care standards and protocols within the area of expertise.

Professional Development

  • Demonstrate advanced practice nursing through professional conduct and ethical decision-making.
  • Engage in professional organizations to maintain active participation in the field.
  • Share clinical expertise and research findings through various professional channels.

Eligibility Criteria

Education

Graduation from an accredited MSN or DNP Nurse Midwifery Program.

Certification

National certification as a CNM.

Licensure

Current unrestricted RN license with authorization to practice as a Certified Nurse Midwife.

Experience

Preferred minimum of 1 year of experience as a CNM.

Life Support Certifications

BLS and NRP certifications are required.

Physical and Mental Requirements

  • Ability to articulate and adhere to the scope of practice for Nurse Practitioners in North Carolina.
  • Capability to independently seek resources and collaborate to address complex issues.
  • Effective communication skills with patients, families, and healthcare teams.
  • Proficient in using sensory and cognitive functions for health assessments and documentation.
