Role Overview:

The Clinical Nurse Coordinator (CNC) is a crucial member of the Intermediate Care Unit team, responsible for coordinating patient care and operational activities. This dynamic leader will work closely with interdisciplinary teams to develop and implement individualized care plans, monitor patient progress, and address any concerns or issues that arise during treatment.

Essential Duties:
- Develop, implement, and evaluate patient care plans in collaboration with the nursing team and other healthcare professionals
- Facilitate effective communication among staff members, patients, and families to ensure smooth transitions and care delivery
- Identify and address areas for quality improvement, proposing solutions and implementing changes as necessary
