At MJHS, we are dedicated to providing exceptional care to our patients and their families. We are seeking a compassionate and skilled Hospice Care Planner to join our team.

Job Summary

The Hospice Care Planner will work closely with our medical center team to evaluate and establish a plan of care for patients referred for home health services. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Responsibilities
  • Evaluate and establish a plan of care for patients referred for home health services
  • Facilitate admission of referrals upon discharge from the hospital to a home care program that meets the needs of the patient and family members
  • Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center
  • Work closely with the business development and marketing team to develop and implement marketing strategies to promote our home care services
Qualifications
  • Licensed as a Registered Professional Nurse in NYS
  • Bachelor's Degree preferred
  • Minimum of two years experience in home care nursing with proficiency in physical assessment and interviewing skills
  • Excellent written and verbal communication skills
  • Advanced critical thinking and problem-solving skills
  • Bilingual Spanish/English required in some locations
