Job SummaryThe Hospice Care Planner will work closely with our medical center team to evaluate and establish a plan of care for patients referred for home health services. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
Responsibilities- Evaluate and establish a plan of care for patients referred for home health services
- Facilitate admission of referrals upon discharge from the hospital to a home care program that meets the needs of the patient and family members
- Develop and maintain positive relationships with physicians, social workers, case managers, and discharge planners within the medical center
- Work closely with the business development and marketing team to develop and implement marketing strategies to promote our home care services
- Licensed as a Registered Professional Nurse in NYS
- Bachelor's Degree preferred
- Minimum of two years experience in home care nursing with proficiency in physical assessment and interviewing skills
- Excellent written and verbal communication skills
- Advanced critical thinking and problem-solving skills
- Bilingual Spanish/English required in some locations
