Job Title: Primary Care Physician

Key Responsibilities:
  • Provide comprehensive assessments and coordinate care with the interdisciplinary team to develop and implement individualized care plans.
  • Oversee the care of participants in nursing homes and hospitals, including admits and discharges for acute, short-term, and long-term placement.
  • Collaborate with community-based physicians and the interdisciplinary team in discharge planning from acute or long-term settings.
  • Participate in the interdisciplinary team, communicating participant changes, collaborating on care planning decisions, and coordinating 24-hour care delivery.
  • Maintain care of participants in the nursing home, making regular visits as dictated by participant need.
  • Participate in rotating night and weekend call.
Requirements:
  • Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
  • Doctorate Degree.
  • Board Certification – Internal Medicine or Family Medicine or Emergency Medicine or Geriatrics (preferred).
  • Active California Physician License without Restrictions.
  • Reliable means of transportation.
  • Active state CDS Certification and DEA.
  • Must be currently enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal participants.

Salary/Wage base range for this role is $250,000 - $280,000 / year + Bonus + Equity. WelbeHealth offers a competitive total rewards package that includes 401k match, healthcare coverage, and a broad range of other benefits.

Actual pay will be adjusted based on experience and other qualifications.
