Job Description

We are seeking a skilled Nursing Professional to join our team in Community Care, where you will provide high-quality nursing care to patients in their homes across New York City.

About the Role

This is a challenging and rewarding role that requires strong clinical skills, effective communication, and a commitment to delivering exceptional patient care. As a Nursing Professional, you will be responsible for assessing patients' needs, developing individualized care plans, and providing skilled nursing care to promote optimal health outcomes.

You will work closely with patients, families, and other healthcare professionals to coordinate care and ensure seamless transitions between settings. Your expertise will be valued as you contribute to the development of comprehensive care plans, prioritize interventions, and make informed decisions to optimize patient care.

Responsibilities
  • Assess patients' physical, emotional, and social needs to develop personalized care plans.
  • Provide skilled nursing care, including wound management, medication administration, and education on disease and medication management.
  • Collaborate with patients, families, and healthcare teams to ensure coordinated care and achieve desired outcomes.
  • Document patient progress, maintain accurate records, and communicate effectively with colleagues and healthcare providers.
  • Maintain up-to-date knowledge of Federal, State, and local regulations, as well as JCAHO standards for Home Care.
Requirements
  • A valid New York State license and current registration to practice as a Registered Professional Nurse (RN).
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
  • Ability to travel regularly to patient assignments across New York City via public transportation and/or private vehicle.
  • Demonstrated commitment to continued professional development.
