Job Opportunity

At Community Healthcare Network, we are seeking a skilled Full-Time LPN I to join our healthcare team. This individual will play a vital role in delivering exceptional patient care across various centers, working closely with health providers to address the unique needs of patients.

Responsibilities:

  • Collaborate with the healthcare team to ensure high-quality patient services.
  • Administer medications and injections accurately, documenting patient responses and ensuring safety.
  • Conduct patient intakes and perform venipuncture as necessary.
  • Order medications, vaccines, and medical supplies as required.
  • Regularly check emergency equipment and maintain accurate refrigerator temperatures.
  • Manage referrals and follow up on patient medical care, including correspondence.
  • Support providers with procedures and provide clarification on prescribed regimens.
  • Offer health counseling to patients, including HIV education.
  • Update prenatal logs and follow up on abnormal lab results.
  • Maintain nursing competence through ongoing education and professional development.
  • Facilitate related services for patients while respecting confidentiality and privacy.
  • Adhere to Infection Control policies and procedures.
  • Work assigned shifts at alternate locations as needed, according to CHN Policy.
  • Perform other duties as assigned by management.

Requirements:

  • High School Diploma or equivalent required.
  • Current New York State License to practice as a LPN.
  • CPR certification or plan to obtain within six months of employment.

What We Offer:
