We are seeking a skilled Medical Social Worker to join our team at Heart of Hospice, a part of LHC Group. As a Medical Social Worker, you will play a vital role in providing medical social services to patients and their families, helping them navigate the complexities of healthcare and find support in times of need.
Responsibilities- Provide medical social services to patients and their families, including assessing their social and emotional needs and developing coping mechanisms.
- Collaborate with the healthcare team to develop a comprehensive plan of care that addresses the patient's physical, emotional, and social needs.
- Conduct assessments and develop plans to address bereavement needs, including establishing a plan of care that addresses bereavement needs with clear delineation of services to be provided and frequency of service delivery.
- Supervise the provision of bereavement services and instruct healthcare team members on community resources available to assist patients.
- Master's Degree from a school of Social Work accredited by the Council of Social Work Education.
- One year of social work experience in a healthcare setting.
- Current CPR certification required.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
