Job Summary:
HealthCare Support is seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will be responsible for providing high-quality patient care and managing complex cases.
Responsibilities:
* Provide patient-centered care and develop individualized care plans
* Conduct thorough assessments and evaluations to identify patient needs
* Develop and implement effective care plans to meet patient goals
* Collaborate with healthcare teams to ensure seamless care transitions
* Monitor patient progress and adjust care plans as needed
Requirements:
* Current RN licensure in the state
* BLS certification from the American Heart Association
* 1+ year of experience in case management
