Key Responsibilities- Case Management: Conduct client intake interviews to determine eligibility for our program, develop care plans, and provide referrals to medical and support services.
- Client Education: Provide education and support to clients regarding their disease, including HIV/AIDS.
- Resource Coordination: Identify program resources available to clients and arrange and coordinate services, including linkage to funded and non-funded entities.
- Advocacy: Provide application information and advocacy to clients to access benefits such as Medicare, Medicaid, and local resources.
- Education: Bachelor's or Master's degree in social or behavioral science or related health specialty.
- Experience: One year of progressively more responsible full-time experience in social services or medical/clinical setting.
