Option Care Health is seeking a highly skilled Clinical Pharmacist to join our team. As a Clinical Pharmacist, you will be responsible for providing safe and appropriate pharmacy services to our patients in accordance with our policies and procedures.

Key Responsibilities:
  • Determine the suitability of individual patients for home care services.
  • Collect and organize patient-specific information needed to determine suitability for home care.
  • Assess patients' suitability for home care in accordance with Option Care policies.
  • Design, recommend, monitor, and evaluate patient-specific pharmacy care plans.
  • Provide medication-use education to Option Care staff, other professionals, home care patients, and/or their caregivers.
Requirements:
  • Graduate of an accredited School of Pharmacy.
  • Current, active Pharmacy license as granted by the State Board of Pharmacy.
  • At least one year of experience in sterile IV admixture procedures, or knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM).
