Behavioral Health Consultant Job Description

Erie Family Health Center is seeking a skilled Behavioral Health Consultant to join our team. As a Behavioral Health Consultant, you will provide high-quality behavioral health services to patients in a primary care setting.

Main Responsibilities:
  • Provide PCP-initiated consultation services, including differential diagnosis, screenings, psychoeducation, brief intervention, and referral for further treatment.
  • Maintain a caseload and provide individualized mental health assessments, treatment, and counseling as appropriate.
  • Consult with primary care providers to enhance understanding of patients, provide decision support for treatment planning, and assist in the implementation and monitoring of biopsychosocial treatment plans.
  • Coordinate care with health center primary care staff, complete psychiatric intake evaluations, and refer patients to psychiatric providers as needed.
  • Participate in crisis intervention and emergency response as needed.
Requirements:
  • Master's degree in a Behavioral Health discipline (e.g., Social Work, Psychology, Mental Health Counseling, etc.) with a valid Illinois State Licensure as appropriate to discipline.
  • Prior work experience as a LCSW or LCPC in a medical setting or Federally Qualified Health Center preferred.
  • Cultural competency to work in a low-income, minority environment.
  • Ability to maintain confidentiality and trust of clients.
