About the Role

We are seeking a highly skilled and compassionate Home Health RN Case Manager to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in providing personalized, comprehensive home care to patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization.

Key Responsibilities
  • Provide admission, case management, and follow-up skilled nursing visits for home health patients.
  • Administer on-going care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
  • Coordinate appropriate care, encompassing various healthcare personnel, including Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
  • Report patient care/condition/progress to patient's physician and Clinical Manager on a continuous basis.
  • Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
Requirements
  • Diploma, Associate, or Bachelor Degree in Nursing.
  • A minimum of one year of nursing experience preferred.
  • Strong med surg, ICU, ER, acute experience.
  • Home Health experience is a plus.
