Physician - Hospice and Palliative Care Specialist

Job Summary: We are seeking a skilled Physician to join our Hospice and Palliative Care team. As a Hospice and Palliative Care Physician, you will work collaboratively with our interdisciplinary team to provide high-quality medical care to patients with terminal illnesses.

Key Responsibilities:

  • Collaborate with the interdisciplinary team to create and implement comprehensive care plans that meet the unique needs of each patient.
  • Certify patients for hospice care and ensure that they meet the necessary eligibility criteria.
  • Provide medical guidance and support to patients, families, and caregivers, and educate them about hospice services and end-of-life care.
  • Work closely with the hospice medical director to review and update patient care plans, and ensure that medical needs are being met.
  • Provide consultation and support to the interdisciplinary team, attending physicians, and other colleagues on medical eligibility, symptom management, and other issues related to hospice care.

Requirements:

  • MD or DO degree
  • Experience in hospice and palliative care
  • Strong communication and interpersonal skills
  • Ability to work collaboratively as part of a team
