Position Overview:

The Healthcare Panel Coordinator is essential in overseeing patient management and facilitating smooth clinic operations. This role involves close collaboration with healthcare professionals and support staff to ensure exceptional patient care.

Key Responsibilities:

  • Report directly to the Panel Specialist Supervisor while collaborating with the Clinical Team Supervisor and healthcare providers.
  • Work autonomously and as part of a multidisciplinary team to uphold proactive healthcare for a designated patient panel.
  • Provide regular updates to the care team regarding preventative health measures.
  • Utilize quality data dashboards to identify patients needing follow-up and coordinate necessary healthcare maintenance.
  • Manage incoming communications via MyChart and respond to patient inquiries effectively.
  • Oversee laboratory service follow-ups and ensure proper documentation for medications and prescriptions.
  • Prepare and manage patient charts, ensuring accuracy in medication reconciliation and health records.
  • Facilitate communication with external healthcare providers and coordinate care plans.
  • Administer clinical quality improvement initiatives and track diagnostic test results.
  • Deliver basic clinical services, including medication administration and patient education.
  • Ensure compliance with electronic health record (EHR) protocols and maintain accurate patient documentation.
  • Provide coverage for other Panel Specialists as needed and participate in staff meetings.

Qualifications:

  • High school diploma or equivalent required; Clinical Medical Assistant certification from an accredited institution is mandatory.
  • A minimum of three years of recent experience in a medical assistant role is essential.
  • BLS certification is required.
  • Bilingual proficiency in English and Spanish is strongly preferred.
  • Familiarity with community health services and medical data systems is advantageous.
  • Excellent communication skills are necessary for effective patient outreach and documentation.
  • Ability to work in a fast-paced environment while maintaining professionalism and confidentiality.
