Position Summary: The Housing First Team Leader is responsible for delivering direct case management assistance to individuals experiencing chronic homelessness while overseeing the operational aspects of the Housing First Program.

Key Responsibilities:

  • Supervise and mentor assigned Housing First Case Managers.
  • Respond collaboratively with Case Managers during business hours for crisis situations and participate in the on-call schedule.
  • Work alongside the Housing First Program Manager to pinpoint areas for program enhancement and development.
  • Assess clients' ability to make independent living choices and evaluate risks to clients and other residents at housing sites.
  • Conduct thorough client interviews and assessments, making necessary collateral contacts and consulting with other service providers.
  • Identify client needs to ensure comprehensive service delivery.
  • Assist clients in finding suitable housing options.
  • Regularly maintain contact with clients through home visits, phone calls, and office meetings.
  • Facilitate transportation for clients to essential appointments and services.
  • Coordinate and advocate for necessary support services and resources for each client.
  • Collaborate effectively with landlords and property managers.
  • Maintain precise case records and timely updates in internal and external databases.
  • Develop and sustain strong relationships with community agencies that support the homeless population.
  • Uphold client confidentiality and adhere to all program policies and procedures.
  • Establish and maintain professional boundaries with clients.
  • Ensure the safety and well-being of clients, reporting any concerns to the supervisor.
  • Promote a respectful and dignified treatment of all clients.
  • Participate in departmental and organization-wide meetings, collaborating with staff across various Action Inc. programs.
  • Act in alignment with the organization's values of teamwork, collaboration, and mutual respect, complying with all relevant policies and regulations.
  • Perform additional job-related duties as required.

Required Skills & Qualifications:

  • Proficient in Microsoft 365, including Outlook and the Office suite.
  • Ability to effectively utilize internet resources and maintain digital records.
  • Strong organizational, interpersonal, and communication abilities.
  • Reliable, responsible, and highly organized with a proactive approach.
  • Skilled in networking and collaborating with various resources and organizations.
  • Excellent interpersonal skills, capable of managing sensitive situations with discretion.
  • Commitment to integrity, professionalism, and confidentiality.
  • Strong time management skills with a proven track record of meeting deadlines.
  • Critical thinking and problem-solving abilities from a humanistic perspective.
  • Experience working with clients in crisis and those dealing with substance use issues.
  • Ability to apply effective de-escalation techniques and ensure personal safety.
  • Compliance with pre-employment background checks.
  • Proficiency in a second language is a plus.

Education & Experience:

  • Bachelor's degree in Human Services or a related field, or four years of relevant experience.
  • Valid Driver's License and reliable transportation are required.

Physical Requirements:

  • Ability to lift, move, and carry up to 15 pounds.
  • Capability to visit clients in their homes and assist with transportation to various locations.
  • Prolonged periods of sitting at a desk and working on a computer.
