Job Summary

We are seeking a skilled Health Educator to join our team at Catholic Charities Family Community Services. The successful candidate will provide education to clients living with HIV/AIDS about HIV transmission and risk reduction, as well as support ongoing communication amongst program staff and ensure a trauma-informed care environment.

Key Responsibilities
  1. Screen clients for health education needs, create health education plans, and complete intake and assessments for clients enrolled in health education.
  2. Conduct assessments and health education plans in areas such as HIV education, risk reduction, treatment as prevention, medical treatment information, adherence counseling, and monitoring.
  3. Develop health education curricula and facilitate education groups to increase HIV+ individuals' independence in medical care activities.
  4. Maintain an individual caseload of HIV+ clients who need specialized attention in areas such as HIV and treatment adherence education, medication adherence, education on healthcare coverage options, and health literacy.
  5. Assist clients in engaging with care and treatment to improve health outcomes and achieve viral suppression.
  6. Address and remove barriers to enable client access to necessary healthcare components, including mental health and substance use services.
  7. Maintain high-quality and timely documentation in electronic databases, including AIRS and Welligent.
  8. Complete and submit encounters in electronic database entries to supervisors weekly for accuracy.
  9. Meet with clients and maintain telephone availability based on client need.
  10. Recognize and support diversity through individual strengths.
  11. Work collaboratively with the SS team to engage clients in care, treatment, and supportive services, with a client-centered goal of viral suppression and self-management.
  12. Conduct and/or participate in multidisciplinary case conferences, coordinate services and referrals with service and clinical providers to facilitate client engagement and retention in care.
  13. Participate in relevant Agency meetings and/or trainings.
  14. Actively pursue professional development through reading, supervision, in-service training, and attendance at conferences and workshops.
Requirements
  • Associate degree, B.A., or B.S. preferred, with 2 years' experience working in the field of HIV/AIDS, behavioral health, substance abuse, health education, or other chronic illnesses.
  • At least 2 years of experience.
  • Ability to prioritize assignments, plan, and complete work projects with minimal direction.
  • Ability to work efficiently and effectively, meet deadlines, and work under pressure.
  • Excellent verbal and written skills.
  • Ability to maintain confidentiality.
  • Ability to demonstrate behaviors and attitudes supporting the Agency's mission, philosophy, and policies.
  • Willingness and ability to foster Agency and program-wide cooperation and teamwork through positive and constructive communication techniques.
  • Possession of a valid NYS Driver's license and use of a registered and reliable vehicle.
