Job Summary

Catholic Charities Family and Community Services is seeking a skilled Health Educator to join our team. As a Health Educator, you will provide education to clients living with HIV/AIDS about HIV transmission and how to reduce the risk of HIV transmission.

Key Responsibilities
  • Screen clients for health education needs and create health education plans
  • Conduct assessments and health education plans in areas such as HIV education, risk reduction, treatment as prevention, medical treatment information, and adherence counseling
  • Develop health education curriculum and facilitate education groups to HIV+ individuals
  • Maintain an individual caseload of HIV+ clients who need specialized attention in areas such as HIV and treatment adherence education, medication adherence, education on health care coverage options, and health literacy
  • Assist with engaging clients into care and treatment to improve their health outcomes and achieve viral suppression
  • Address and work to remove barriers to enable client access to all necessary components of health care, including but not limited to mental health and substance use services
  • Maintain high-quality and timely documentation in electronic databases
Requirements
  • Associate degree, B.A. or B.S. preferred, with 2 years' experience working in the field of HIV/AIDS, behavior health, substance abuse, health education, or other chronic illnesses
  • At least 2 years of experience
  • Ability to prioritize assignments, plan, and complete work projects with minimal direction
  • Ability to work efficiently and effectively and meet deadlines
  • Excellent verbal/written skills
  • Ability to maintain confidentiality
  • Ability to demonstrate behaviors and attitudes which support agency mission, philosophy, and policies
  • Willingness and ability to foster agency and program-wide cooperation and teamwork through use of positive/constructive communication techniques
  • Possession of a valid NYS Driver's license and use of a registered and reliable vehicle
