Job Summary

Catholic Charities Family and Community Services is seeking a skilled Health Educator to join our team. As a Health Educator, you will play a vital role in providing education and support to clients living with HIV/AIDS.

Key Responsibilities
  • Develop and implement health education plans for clients, focusing on HIV transmission, risk reduction, and treatment adherence.
  • Conduct assessments and provide individualized education to clients, addressing their unique needs and concerns.
  • Facilitate education groups and workshops to promote health literacy and independence in medical care activities.
  • Maintain accurate and timely documentation in electronic databases, ensuring compliance with agency standards.
  • Collaborate with the Social Services team to engage clients in care and treatment, promoting viral suppression and self-management.
  • Participate in multidisciplinary case conferences and coordinate services with clinical providers to ensure comprehensive client care.
Requirements
  • Associate degree or higher in a related field, with 2 years of experience working in HIV/AIDS, behavioral health, or a related field.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to prioritize assignments, plan, and complete work projects with minimal direction.
  • Valid NYS Driver's license and reliable vehicle.
