We are seeking a skilled Occupational Therapist to join our team in Upper Marlboro, MD. As a Home Health Occupational Therapist, you will work with patients in their homes, providing essential one-on-one care and supporting their families.
Responsibilities- Assess and analyze patient care needs, establishing mutual goals with the patient and/or caregiver for a comprehensive plan of care.
- Ensure the patient's environment is safe and suitable for the delivery of effective care.
- Identify therapy diagnosis/patient care problems and interventions, making additions or changes as necessary.
- Implement services and programs related to Occupational Therapy, ensuring orders are carried out in a professional manner.
- Document additions and/or changes to the patient plan of care, re-evaluating progress and making changes as necessary.
- Provide patient and caregiver education relating to diagnosis, medications, infection control, and safety measures.
- Communicate with other disciplines to ensure collaboration, coordination of care, and enhance patient outcomes.
- Graduate of an Occupational Therapist Education program approved by ACOTE.
- Current Occupational Therapist licensure in State and American Heart Association CPR certification required.
- Two (2) years of experience in adult occupational therapy, home health experience preferred.
