Job Summary

We are seeking an Occupational Therapist to join our team at VNA of Maryland, a part of LHC Group. As an Occupational Therapist, you will play a vital role in helping patients achieve their goals and improve their quality of life.

About the Role

The Occupational Therapist will be responsible for assessing and evaluating patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment. You will work closely with the healthcare team to develop and implement individualized plans of care that meet the unique needs of each patient.

Responsibilities

• Conduct thorough assessments of patients' functional status and occupational therapy needs
• Develop and implement individualized plans of care that meet the unique needs of each patient
• Provide evidence-based occupational therapy interventions to patients
• Collaborate with the healthcare team to ensure seamless care coordination
• Document patient progress and outcomes in accordance with regulatory requirements

Requirements

• Current Occupational Therapy licensure in the state of practice
• Current CPR certification
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
