Home Support Specialist

New Horizons Rehabilitation is seeking a skilled Home Support Specialist to provide assistance to adults with intellectual and developmental disabilities in their homes and communities.
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to foster positive relationships with the individuals they support.
The Home Support Specialist will work collaboratively with the individuals to develop skills, practice independence, and explore opportunities, while providing support and guidance as needed.
Key Responsibilities:
- Assist individuals with daily living tasks and activities
- Develop and implement personalized support plans
- Provide emotional support and encouragement
- Collaborate with other support staff and professionals as needed
Requirements:
- High school diploma or equivalent required
- Valid driver's license and reliable transportation
- First Aid/CPR certification
- Ability to work independently and as part of a team
