Job Summary

As a skilled nursing professional, you will play a vital role in delivering high-quality patient care in the comfort of their own homes. Our team at SSM Health is dedicated to providing compassionate and personalized care to patients and their families. We are seeking a highly motivated and experienced RN to join our Home Health and Hospice team.

Responsibilities
  • Assess and coordinate patient care needs with the healthcare team.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Provide skilled nursing care, including wound care, IV therapy, and medication management.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions.
  • Document patient progress and communicate effectively with the healthcare team.
Requirements
  • Current RN licensure in the state of employment.
  • Basic Life Support (BLS) certification.
  • Valid driver's license and reliable transportation.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
