We are seeking a skilled Licensed Practical Nurse to join our team at NHC HomeCare Cookeville. As a LPN, you will provide high-quality nursing care to patients in their homes, working under the direction of a Registered Nurse.
Key Responsibilities- Provide skilled nursing services as dictated by the plan of care and document according to policy/procedures.
- Instruct patients/caregivers in various aspects of patient care and disease management, documenting their comprehension and compliance.
- Observe patient response to care/teaching and make recommendations regarding revision of plan for patient care to the Registered Nurse.
- Record and report symptoms, vital signs, and other observations that may be indicative of care plan reactions and changes.
- Follow and teach infection control and safety policies and procedures.
- Participate in on-going care coordination and maintain positive relationships with referral sources, physicians/other clinicians, patients/families, and other partners.
- Licensed Practical Nurse in the state where HomeCare agency is located.
- Graduate of a state-approved vocational education program.
- One year clinical experience is preferred.
