Responsibilities
  • Provide Direct Patient Care: Assess patient needs, develop and implement individualized care plans, and provide ongoing evaluation and adjustment of care as needed.
  • Collaborate with Healthcare Team: Work closely with physicians, social workers, and other healthcare professionals to ensure comprehensive patient care.
  • Coordinate Case Management: Identify patient needs, develop and implement care plans, and coordinate services to ensure optimal patient outcomes.
  • Communicate Effectively: Maintain open and effective communication with patients, families, and healthcare team members to ensure seamless care coordination.
  • Document Patient Information: Accurately and thoroughly document patient information, including assessment, care plans, and progress notes.
  • Participate in Quality Improvement: Contribute to quality improvement initiatives and participate in ongoing education and training to enhance patient care and outcomes.
Requirements
  • Graduate of an Accredited School of Professional Nursing: Bachelor's degree in nursing preferred.
  • Licensure and Certifications: Current RN licensure and Basic Life Support (BLS) certification required.
  • Automotive Insurance: Valid driver's license and automotive insurance required.
