We are seeking a skilled Registered Nurse to join our Home Health team in Ebensburg, PA. As a Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver comprehensive care.
Responsibilities- Conduct initial and comprehensive nursing evaluations to determine primary focus of care and develop individualized plans of care.
- Coordinate total plan of care with other healthcare professionals, ensuring continuity of patient care and timely documentation.
- Provide patient education and support to promote optimal health outcomes.
- Collaborate with physicians and other healthcare professionals to ensure seamless care transitions.
- Ensure patients meet home health eligibility and medical necessity guidelines.
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
