C&S Care Management will hire the professional that has a passion for elder care that leads to 5 star outcomes and customer satisfaction Our RN Care Manager leader will function as a specialist in home care, a liaison for clients, an advocate for families, an innovative designer of personalized care plans and so much more.

Job satisfaction and career fulfillment are wrapped up in this position. The big PLUS we offer is that our RN Owner & Practitioner will, personally, mentor the person selected, sharing her 20 years of experience

Requirements:

  • A Baccalaureate Degree or Associates Degree in Nursing or a Master's Degree in Nursing
  • Current professional license for the State of Texas
  • Two or more years of professional experience working with the elderly or disabled in a related healthcare field of a private or non-profit agency, facility or community
  • Computer proficient.

Preferred experience:

  • Care Management, Home Health

Responsibilities:

Assessments:

Conducts comprehensive assessments of each client, evaluating the physical, psychological, financial and environmental strengths, weaknesses and concerns as indicated.

Care Plan Development:

Develops personalized care plans utilizing available resources and support systems.

Advocacy:

Advocates whenever needed on behalf of the client including supportive listening and professional case conferences.

Staffing Services:

Refers and monitors appropriate staffing services when needed.

Care Coordination:

Makes regular home or facility visits, phone communication to service providers.

Case Supervision:

Weekly review of all cases.

Placement and Relocation:

Visiting communities, residential care homes and healthcare facilities to maintain an ongoing resource.

Making all necessary arrangements when relocation is requested for clients to residential/care facilities or nursing homes for private pay or funded programs (Ex: Medicare, Medicaid, VA)
