Job Overview

The Street Medicine Coordinator position at Special Service for Groups, Inc - HOPICS is a vital role in providing comprehensive healthcare services to individuals experiencing homelessness.

About the Job

We are seeking a highly motivated and detail-oriented individual to coordinate between our Street Medicine team and Outreach teams to provide street-based health-care services. The successful candidate will assist with determining MediCal eligibility, Managed Care Plan enrollment, data collection, and program support.

Key Responsibilities

  • Maintain accurate databases of E6 Outreach clients with possible health conditions who could benefit from street-based primary care services.
  • Assist Outreach teams with determining insurance eligibility and communicate daily with clients and the subcontracted Street Medicine team.
  • Critical thinking and problem-solving skills are essential to effectively schedule Street Medicine visits and maximize efficiency.
  • Coordinate with Harm Reduction teams to arrange primary care visits from the Street Medicine team and provide daily documentation and data entry into HMIS and other systems as required.
  • When necessary, assist the Street Medicine team in the field to ensure smooth coordination with clients.
  • Identify system gaps to contribute to ongoing program quality improvement initiatives.
  • Collaborate with IT Department to support administrative and data-related functions.
  • Support special outreach projects as assigned by the Program Manager or Director.
  • Foster professional relationships with community organizations, local authorities, and stakeholders.

Qualifications

  • Some post-secondary education credits required.
  • Proficiency in Excel and Word.
  • Excellent verbal and written communication skills.
  • Ability to work with diverse client populations, including those experiencing homelessness.
  • Neat and professional appearance, attention to detail, and punctuality required.
  • Valid CA Driver's License and reliable transportation needed.

Working Conditions

This position requires working in various environments, including homeless encampments, and interacting with individuals who may be aggressive or hostile. Ability to maintain a professional demeanor under pressure and deal with traumas triggered by personal experiences.

Compensation

Salary: $25-$28 per hour based on experience and qualifications.
