About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our team at Amedisys, Inc., a leading provider of home health, hospice, and high-acuity care. As a Registered Nurse, you will play a vital role in delivering patient-centered care to individuals in the comfort of their own homes.

Responsibilities
  • Coordinate and oversee all aspects of patients' home care, ensuring seamless transitions and effective communication with care teams.
  • Collaborate with patient care teams to develop and implement personalized care plans, addressing the unique needs and goals of each patient.
  • Educate families on patient condition, needs, and care, empowering them to take an active role in their loved one's recovery.
  • Perform head-to-toe physical assessments, checking vital signs and managing medications to ensure optimal patient outcomes.
  • Administer and maintain catheters, give injections, IV therapy, or tube feedings as needed, and complete documentation in a timely and accurate manner.
  • Supervise LPNs and HHAs, providing guidance and support to ensure high-quality patient care.
  • Regularly communicate patient progress to the clinical manager, physician, and care team, ensuring a collaborative approach to patient care.
Requirements
  • One year of clinical experience as a Registered Nurse (RN), with approval by VP clinical for less than 1 year of experience.
  • Current RN license, specific to the state(s) you are assigned to work.
  • Minimum six months home health experience.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
  • Able to assess patient status and identify requirements relative to age-specific needs.
  • Familiarity with general use and functions of the computer, including user names and password concepts, internet, email, navigation of computer desktop, and online HR services.
