- Conduct thorough assessments, develop, and implement individualized care plans to address patients' unique needs.
- Collaborate with healthcare professionals to ensure seamless care coordination and effective communication.
- Provide education and training to patients and their families on disease management, medication adherence, and healthy lifestyle habits.
- Document patient progress, maintain accurate records, and communicate with the care team to ensure continuity of care.
- Graduate of an accredited LPN program or equivalent.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN in a clinical setting, preferably in home health or hospice care.
