Key Responsibilities:- Provide professional nursing services under the direction of a RN, adhering to state regulations, healthcare standards, and CenterWell Home Health policies.
- Develop and implement individualized care plans, focusing on patient-centered goals and outcomes.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and effective communication.
- Conduct thorough assessments, identify patient needs, and develop targeted interventions to promote optimal health outcomes.
- Document patient progress, maintain accurate records, and communicate effectively with healthcare teams.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid drivers license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
