Quality Improvement Coordinator - Hospice Care

At Community Hospice of Northeast Florida, we are seeking a highly skilled Quality Improvement Coordinator to join our team. This role is responsible for planning, gathering, monitoring, analyzing, and reporting data that represents our agency's performance improvement and patient safety monitors and outcomes for regulatory and internal purposes.

Key Responsibilities:
  • Assist in the development of all QAPI activities and education with appropriate data collection, aggregation, analysis, taking action, and reporting of results according to our QAPI plan.
  • Review patient medical records for compliance with federal, state, and local laws, accreditation standards, and agency policies and guidelines.
  • Chair the QAPI committee, prepare QAPI reports/minutes of meetings, and forward reports according to the QAPI plan.
  • Responsible for weekly and monthly chart compliance audits.
  • Understand and promote principles of continuous QAPI.
  • Responsible for the orientation of new staff to our QAPI program.
  • Assist in the training and education needs of staff.
  • Assist in the preparation and implementation of policies and procedures that meet Medicare and Medicaid accreditation standards, and state and local laws.
  • Participate in studies and other administrative functions assigned.
  • Serve as a resource to employees.
  • Investigate and report any problem related to patient care and/or employee well-being.
  • Immediately report any incident, accident, lost articles, or unusual occurrence.
  • Attend pertinent continuing education programs other than routine in-services and share information with staff.
  • Perform other duties as assigned by the Executive Director.
Qualifications:
  • Graduate of an approved school of professional nursing and currently licensed in Alabama.
  • Minimum of 5 years nursing experience, registered nurse experience preferred.
  • Minimum of 5 years hospice experience.
  • Minimum of 1-year quality assessment/performance improvement (QAPI) in a hospice setting.
  • Ability to exercise initiative and independent judgment.
  • Ability to enlist cooperation of many people to perform/achieve a common goal.
  • Proficiency in Microsoft Office.
