Responsibilities- Assess patients' risk factors and identify those who would benefit from care coordination services.
- Collaborate with the clinical team to develop and implement effective care plans.
- Monitor and manage length of stay, ensuring patients receive the right level of care.
- Lead and facilitate multidisciplinary patient care conferences.
- Communicate with patients and families about their plan of care.
- Identify and document potentially avoidable days and report over and underutilization.
- Current Florida RN license and BLS/Healthcare Provider certification.
- Three years of experience in chronic disease management, care management, care coordination, utilization management, or acute clinical care.
