Job Summary
We are seeking a highly motivated Clinical Case Manager to join our team in West Warwick, RI. The successful candidate will work closely with clients, families, and healthcare professionals to provide comprehensive support and guidance.
Key Responsibilities
The Clinical Case Manager will be responsible for:
- Assessing client needs and developing personalized recovery plans;
- Collaborating with healthcare providers, social workers, and other stakeholders to ensure seamless care coordination;
- Providing education, advocacy, and support to clients and their families on mental health issues, substance abuse, and related topics;
- Maintaining accurate records and documentation of client interactions and progress;
- Scheduling appointments and ensuring timely completion of tasks and responsibilities;
- Participating in regular team meetings and contributing to the development of new initiatives and strategies;
- Staying up-to-date with relevant research, best practices, and industry developments to enhance service delivery and outcomes;
- Performing other duties as assigned by management.
Requirements
To be considered for this role, candidates must have:
- A Bachelor's degree in Human Services or a related field;
- A minimum of 2 years of experience in case management, preferably working with clients with disabilities;
- Basic computer literacy skills and proficiency in software applications such as Microsoft Office;
- A valid driver's license and reliable transportation;
- The ability to work effectively in a fast-paced environment and prioritize multiple tasks simultaneously;
- Excellent communication, interpersonal, and problem-solving skills;
- A passion for delivering exceptional customer service and making a positive impact in the lives of others.
Salary and Benefits
The salary range for this position is $52,000-$65,000 per year, depending on experience. In addition to a competitive salary, we offer a comprehensive benefits package, including medical, dental, and vision insurance, paid time off, and opportunities for professional growth and development.
Location
