Job SummaryWe are seeking a highly skilled and compassionate Primary Care Physician to join our team at ChenMed. As a Primary Care Physician, you will be responsible for providing exceptional patient care, leading a care team, and contributing to the growth and success of our organization.
Key Responsibilities- Provide high-quality, patient-centered care to a panel of up to 450 patients, with a focus on acute and chronic illnesses in older adults.
- Lead a care team consisting of care promoters, care facilitators, and care coordinators to ensure seamless patient care and outcomes.
- Develop and implement individualized care plans, taking into account patients' unique needs, preferences, and goals.
- Collaborate with specialists, hospitalists, and other healthcare professionals to ensure comprehensive patient care.
- Participate in quality improvement initiatives, patient safety programs, and risk management activities to ensure the highest standards of care.
- Stay up-to-date with the latest evidence-based medicine, clinical guidelines, and best practices in primary care.
- Develop and maintain strong relationships with patients, families, and caregivers to ensure patient satisfaction and engagement.
- Contribute to the growth and success of ChenMed by participating in recruitment efforts, mentoring new physicians, and sharing best practices.
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
- Current, active State Medical License for the state(s) in which you will be working.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty (Board Eligibility required).
- Current DEA number for schedule II-V controlled substances.
- Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross (ARC) within the first 90 days of employment.
