Primary Care Physician

We are seeking a highly skilled and compassionate Primary Care Physician to join our team at Oak Street Health. As a Primary Care Physician, you will be responsible for providing high-quality, patient-centered care to our Medicare patients in a community-based setting.
Job Summary:The Primary Care Physician will be responsible for assessing and diagnosing patients, overseeing primary care, prescribing and administering pharmaceutical treatments, and maintaining accurate patient records. You will work collaboratively with our care team to ensure positive health outcomes and provide exceptional patient experiences.
Responsibilities:- Assess and diagnose patients in a community-based setting
- Oversee and direct primary care services
- Prescribe and administer pharmaceutical treatments and medication
- Maintain accurate patient electronic medical records
- Collaborate with regional and central leadership to meet health quality goals
- Work with Practice Managers to direct and manage the care team
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) Graduate
- Internal Medicine or Family Medicine Board Certification (or board eligible)
- Active, non-probationary, unrestricted State License
- Active DEA license
- USA work authorization
- Fellowship training in Geriatrics
- Experience practicing medicine among Geriatric populations
- Experience operating in a Value-Based Healthcare Model
- Experience working in a collaborative setting to ensure positive health outcomes
