Job Description


The Patient Care Coordinator plays a vital role in delivering high-quality patient care and ensuring a positive experience for our patients and families. This professional caregiver will work collaboratively with our interdisciplinary team to provide safe, effective, and compassionate care.


Key Responsibilities:



  • Provide direct patient care under the supervision of registered nurses, including assessing patient needs, developing care plans, and implementing interventions.
  • Collaborate with the healthcare team to ensure seamless transitions of care and effective communication among all stakeholders.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
  • Maintain accurate and detailed records of patient interactions, care plans, and progress notes.
  • Support and mentor new staff members to promote a culture of excellence and teamwork.

Required Skills and Qualifications


To be successful in this role, you will need:



  • A valid Registered Nurse (RN) license or current enrollment in a nursing program.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and colleagues.
  • Strong problem-solving and critical thinking skills, with the ability to prioritize tasks and manage competing demands.
  • Able to lift 30-50 pounds and work in a fast-paced environment.
