As a Nursing Care Coordinator, you will oversee the care provided by LPNs, Certified Nursing Assistants, and other team members dedicated to our residents.

Responsibilities include:

  • Coordinating nursing care for residents, ensuring that all are assigned staff capable of delivering appropriate care.
  • Administering safe and precise medication-related interventions.
  • Assessing residents' health and promptly notifying physicians of any changes in status while implementing new orders.
  • Developing individualized care plans based on thorough assessments and executing nursing care.
  • Selecting and implementing suitable nursing interventions to stabilize residents' conditions and prevent complications.
  • Contributing to residents' assessments (MDS/CAA's) and the creation of care plans.
  • Demonstrating a solid understanding of fundamental nursing practices with a focus on gerontological nursing.

Qualifications:
Additional certification in a nursing specialty is preferred.
