Nurse Coordinator for Acute Care

DaVita Kidney Care is seeking a skilled Registered Nurse to join our team as a Clinical Coordinator for Acute Care. In this role, you will have the opportunity to work in a fast-paced hospital setting, providing high-quality care to patients with end-stage renal failure or chronic kidney disease.

Key Responsibilities:

  • Oversee a team of patients with the support of PCTs to conduct patient observations, measuring stats and machine set up.
  • Deliver care to patients who are often dealing with multiple co-morbidities which require unique treatment plans and the ability to leverage a broad range of nursing skills and knowledge.
  • Build meaningful and long-term relationships with patients and their families in an intimate outpatient setting.
  • Be a part of a team that supports and relies on each other in a positive environment.

Requirements:

  • Current Registered Nurse (RN) license in the state of practice.
  • At least 18 months of registered nursing experience.
  • Dialysis Charge RN readiness approval by clinical Nurse Manager or RN-licensed FA/GFA required.
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) preferred.
