Job Summary

We are seeking a highly skilled and compassionate Psychiatric Nurse to join our team as a Home Health Care Specialist. As a key member of our care team, you will provide high-quality, patient-centered care to individuals in their homes, promoting their physical and mental well-being.

Responsibilities
  • Perform initial nursing evaluations, including behavioral health/psychiatric nursing needs, to identify patient requirements and develop personalized care plans.
  • Observe and evaluate patients' physical and mental status, monitoring for complications and medication side effects, and adjusting treatment modalities as needed.
  • Instruct patients and caregivers on cognitive and technical aspects of care, medication regimes, disease processes, safety interventions, ADL care, coping skills, symptom management, and other interventions.
  • Administer IM medications, perform lab draws, and collect specimens as required.
  • Act as a case manager, coordinating care for patients and communicating regularly with the clinical manager, psychiatrist, and care team.
  • Supervise LPNs and HHAs, ensuring high-quality care delivery and adherence to established protocols.
Requirements
  • Current RN license, specific to the state(s) you are assigned to work.
  • One of the following qualifications to meet CMS requirements for psychiatric nursing in the home care setting:
    • An RN with a master's degree in a mental health nursing or psychiatric specialty.
    • An RN with a bachelor's degree in nursing and one year of recent nursing experience in an acute treatment unit or other psychiatric setting.
    • An RN with a diploma or associate's degree in nursing and two years of recent nursing experience in an acute treatment unit or other psychiatric setting.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
