Job Opportunity

VNA Health Group is seeking a skilled Registered Nurse to join our dedicated Hospice Team in Ocean County.

Key Responsibilities:

  • Assess and evaluate patient needs, developing personalized care plans to ensure optimal outcomes.
  • Collaborate with interdisciplinary teams to provide comprehensive care and support.
  • Provide education and guidance to patients and families, promoting self-care and independence.
  • Monitor patient progress, making adjustments to care plans as needed.
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.

Requirements:

  • RN with Bachelor's degree in Nursing
  • Certification in Hospice Palliative Care Nurse and Critical Care preferred
  • 1+ year of supervised clinical experience as a registered nurse
  • Excellent communication and interpersonal skills
  • Strong computer skills and ability to navigate electronic medical records
